個人情報のお取り扱い

プライバシーポリシー（個人情報保護方針）

大阪ワイズカウンセリング（以下「当サイト」）は、個人情報の保護を重要な責務と考え、適用法令・ガイドラインに基づきその取り扱い方針を定めます。
本ポリシーは、利用目的、第三者提供、安全管理、開示等の手続きおよびCookie等の取り扱いを明示するものです。

プライバシー保護について

個人情報取扱相談者およびお問い合わせの方の個人情報保護を徹底します。
以下、プライバシーポリシーです。

1．個人情報の取得

予約フォーム、お問合わせフォーム、LINEなど問合わせ手段に記載された個人情報を取得します。

2．個人情報の取得目的

相談に必要なものとカウンセラーが判断し、取得します。
取得した個人情報、相談内容は身内であっても第三者に許可なく開示しません。

3．個人情報の内容

取得する個人情報の内容は以下のとおりです。

氏名
性別
年齢
住所（市名）
電話番号
メールアドレス

4．個人情報の利用

取得した個人情報は相談者およびお問合わせの方の権利を損なわないように、管理し、連絡手段としての利用とします。
カウンセリングの録音については、内容を検証するために利用することがあります。（カウンセリング時に録音を望まれない場合、お知らせ下さい）

5. 個人情報訂正および削除

登録者本人とその保護者から個人情報の訂正、削除の申し出があった場合、合理的な範囲内でこれを行ないます。
また、登録者の同意なしに第三者に開示・提供することは一切ありません。

6. 個人情報開示の例外

法令により開示を求められた場合
病気、事故などにより登録者の承認を得ることが困難である場合
相談者本人及び他者の生命が危険にさらされる恐れがある、重大な不法行為を行う可能性があると当社が判断した場合
警察など公的機関から正式に開示を求められた場合、登録者自身の同意なく個人情報を開示・提供することがあります。

7. カウンセリングの録音

原則行っておりません。
カウンセラーが身の危険を感じる場合、精神障害や病気の可能性があると判断した場合など行う場合があります。
この情報管理は上記の6項に従い扱い、相談での発言内容で異なる解釈、被害妄想が出るなど問題が起きる際の証拠判断に使用します。
似た状況で、麻酔でのせん妄による誤解など医療現場でも起きており、導入しています。

当サイトの著作権は株式会社ワイズライフラボ・ジャパンにあります。
当サイトの内容(写真、ロゴ等を含む)を無断で複写・複製することを禁じます。

We thoroughly protect the personal information of clients and inquirers. Below is our privacy policy.

1. Acquisition of Personal Information

We obtain personal information entered in inquiry methods such as reservation forms, contact forms, and LINE.

2. Purpose of Acquisition

We acquire information deemed necessary for consultation by the counselor.

Acquired personal information and consultation details will not be disclosed to third parties, including family members, without permission.

3. Types of Personal Information

We may obtain the following information:

Name, gender, age, address (city), telephone number, and email address

4. Use of Personal Information

We manage personal information so as not to infringe on the rights of clients and inquirers, and use it as a means of communication.

Session recordings may be used to review content. (If you do not wish sessions to be recorded, please let us know.)

5. Correction and Deletion

Upon request from the registrant or their guardian, we will correct or delete personal information within a reasonable scope.

We will not disclose or provide information to third parties without the registrant’s consent.

6. Exceptions to Disclosure

We may disclose personal information without consent when required by law; when obtaining approval is difficult due to illness or accident; when there is a risk to the life of the client or others; when we judge there is a possibility of a serious unlawful act; or upon a formal request from public authorities such as the police.

7. Audio Recording of Sessions

In principle, we do not record sessions. However, recording may occur when the counselor feels at risk, or when a mental disorder or illness is suspected.

Such information is managed in accordance with item 6 above and may be used as evidence in cases of differing interpretations or delusional damage arising from statements made during consultations.

Similar misunderstandings (e.g., delirium due to anesthesia) are known to occur in medical settings, and we have introduced recording for this reason.
